If you would like to schedule an appointment, you may call our main office, and we will be glad to assist you in scheduling your appointment.

Please have your demographic information( medical insurance, driver license numbers, work/home phone numbers and addresses etc.) available at the time you are scheduling your appointment.

We look forward to helping you with your orthopaedic condition. Please bring the following items to your appointment with you: All insurance cards that you may have. If your insurance requires you to have a referral from your primary care physician, you should have this before your visit.

Any studies, such as X-Ray's or MRI's that you may have previously had

If another orthopaedic physician has seen you before, or if you have ever previously had orthopaedic surgery, you should obtain your clinic notes from that physician and your operative reports and bring them with you.

If your injury stems from an injury at work then you should bring all information pertaining to your claim such as: Claim Number, Claim Adjuster, Employers Name, Address and Phone Number, the date of the injury and the exact circumstances surrounding the injury, and any hospitals or other physicians who treated you for the injury

If your injury stems from a motor vehicle accident, you should bring a copy of the liability insurance involved.

If your injury stems from an accident at school, you should have your coach fill out the accident form and submit it to our office.

Please bring a detailed list with accompanying dosages of ALL of your medications you are currently taking

Payment in full is expected at the time of your visit. Exceptions include workers compensation. You may contact our office for a complete list of all insurance plans that we accept. Since most insurance companies do not reimburse the total amount due, you will be responsible for any charges that are not covered under your plan. For your convenience, we accept Visa, MasterCard, cash, or check.

If your injury is a result of a motor vehicle accident or if it is currently involving litigation, you will still be responsible for you bill at the time of service. We do not accept liability claims as compensation.
