Medical insurance plans and payment options
The following is a partial listing of the major Medical Insurance Plans with whom we participate. Since Plans change and new Plans are added, please call our office at 859-344-4400 to confirm our participation in your Plan.
Also, if your Insurance Plan requires a REFERRAL, please arrange for it to be sent to our office prior to your appointment.

Please bring your current Medical Plan INSURANCE CARD with you at the time of your visit.
If we participate in your network, we will bill your Medical Insurance Plan and accept their allowable fee for covered services as full payment, after all applicable co-pays and deductibles. Non-covered services will be billed to the responsible party.
We ask that office co-pays be paid at the time of your visit.
Mastercard, Visa, and American Express cards are accepted.
