Patient Information

Insurance & Referrals

We participate in a variety of healthcare insurance programs which aid in the payment of your medical costs. Our business office will submit an initial claim on your behalf if complete insurance information has been provided to our office. Please be aware that some companies pay a fixed allowance on certain procedures and others pay a percentage of this charge. It is your responsibility to pay any deductible, co-insurance or any other balance left unpaid by your insurance provider.

Most carriers no longer require referrals. We ask that you check with your insurance company and secure one if needed.

New Patients

Please plan to arrive 15 minutes early to the first appointment.

Bring to your appointment:

  • Medications list
  • Allergeries list
  • Insurance card
  • Photo I.D.

On your first visit you will be asked for basic information to establish your medical record and business account. Please bring your current insurance card to this first visit and notify our office of any changes in name, address, phone number or insurance information, as soon as they occur.

Please advise staff of any X-rays, EMG testing or other medical treatment you have received for your condition prior to your visit. It would also be helpful to bring a typed or printed list of your allergies and medications to your appointment.

Appointments

Patients are seen by appointment. We do our best to keep our appointment schedule. However, please understand that not all patients require the same amount of time and emergencies do occur, so some delays are unavoidable. We appreciate your patience.
