Medical insurance plans and payment options

We participate in a variety of healthcare insurance programs which aid in the payment of your medical costs. Our business office will submit an initial claim on your behalf if complete insurance information has been provided to our office. Please be aware that some companies pay a fixed allowance on certain procedures and others pay a percentage of this charge. It is your responsibility to pay any deductible, co-insurance or any other balance left unpaid by your insurance provider. Your insurance carrier may require a referral from your primary care physician in order to receive treatment in our office. Please contact your doctor prior to your appointment to ensure the proper referral paperwork will be received prior to your visit. The following list of carriers currently require a referral from your primary care physician: John Deere Healthcare Advantage/Select, Aetna [Rockwell-Collins], Unity Choice, SecureCare, Blue Choice and Blue Advantage, Iowa Health Solutions, Medipass Medicaid, Tricare Active Duty & Prime Remote.

Appointments for a work related condition and injuries will require a First Injury report to be filed with your employer.
