Medical insurance plans and payment options
Medicaid: Children - 17 years and under; other by physician referral only
Accept assignment on Medicare, patients are responsible for their 20% of Medicare allowable charges and deductibles.
All patients are responsible for their own co-pay at the time of sign-in, per insurance contract. Patients are also responsible for percentage due at time of service if their policy is a traditional plan. If deductible has not been met payment in full is expected at time of service.
Patients with insurance policies that are out of network are responsible for the balance due if their policy does not pay. There are some insurance companies that will pay out of network but some will not. In that case a partial payment is made by their insurance company and they will be responsible for the balance remaining. (Ask about further details.)
Patients with no insurance are asked to pay for services rendered in full at the time services are rendered.
Motor Vehicle Accidents/Liability Accounts:
We do not accept letters of protection from attorneys. We do not get involved in filing ANY insurance. Payment is expected in full from patient for each visit. You will be given a receipt to turn in and be reimbursed by your liability carrier. (Ask about further details.)

Other staff in my practice
When you enter our office you will be greeted by the smiling faces our recptionists. They are part of the highly trained staff we have assembled who work to make your visit go smooth and with as minimal stress as possible. They will first assist you in registering and obtaining any records from other physicians, which can provide background on your case. You will then meet many of the staff as they get you to the exam rooms, make sure all your information is present and take whatever x-rays are needed. Kathryn, the office manager, is always available for any questions you may have.
