Office hours are 8:30 am to 5:00 pm Monday
through Friday. Patients are seen by
appointment only. Please call
978-745-6282 to make an appointment. We
request that all patients under the age of 18 be accompanied by a parent or
legal guardian at the time of their visit.
If this is not possible, the patient must bring a signed written consent
giving the physician permission to treat the patient.
Unforeseen
emergencies and complex patient problems may cause occasional interruptions in
the physicians’ schedule, thereby causing some inconvenience for you. We ask that you understand that this may
occur because we are trying to offer the best personal service and availability
to all of our patients.
Facilities --
Our office has an x-ray facility on premise as well as the ability to
apply casts and splints for acute injuries.
Cortisone Injections and other minor procedures are performed in the
office as well for patient convenience.
Acute Injuries – Appointments are available on
short notice for acute injuries and sudden problems.
Emergencies
– An Orthopaedic Surgeon is available at all times. Should an emergency situation arise, call 978-745-6282. We will do our best to respond promptly to
your orthopaedic problems whenever they occur.
You will be given instructions by the physician on call. By following this procedure, you will avoid
unnecessary delays which often occur in the Emergency Rooms. If you are brought to the Emergency Room,
please request that we be called for your orthopaedic care.
Preparing for Your Office Visit – What to Bring –
It is important that you bring all X-rays and MRIs (including reports of the
X-rays and MRIs) and medical information pertinent to your problem with you at
the time of your visit. Please bring a
list of all your current medications, as well.
If you are insured by an HMO, your are
responsible for obtaining a referral from your primary care physician prior to
your visit. All co-payments are to be
paid at the time of your visit. Please
bring your health insurance identification card with you.
Medical Records – All medical records, including
our X-rays, are the property of Salem Orthopedic Surgeons, Inc. We will release information to another
physician, and insurance company, or an attorney only with your authorization.
We will send
information to a referring at no cost.
Our fee for all other copies and reports will be provided upon request.
Prescriptions – Requests for prescription refills
should be made Monday through Friday from 10:00 am to 2:00 pm. Please have your prescription number and the
pharmacy telephone number available when you call.
