Medical Records

Contact Amy Trujillo for a copy of your medical records from OANNM. You will need to complete a release form. Once you do so, the records should be ready for you to pickup (or will be mailed at your request) within 5-7 business days. Records requested by an attorney will be processed upon receipt of the appropriate fee. Contact Amy at (505) 424-0200, ext. 229.
