Notice of Privacy Practices New Patient QuestionnairePlease take a few minutes to print out and then complete this three-page "New Patient Questionnaire" prior to your visit. Please be sure to bring the form with you and present it to our representative when registering. This should help give you time to gather needed medical information at home, as well as expedite the 'check-in' process upon your arrival.

With a focus on meeting the needs of each patient, our practice employs a wide range of orthopaedic, allied health, and administrative experts. The orthopaedic surgeons on our staff are Board Certified by the American Board of Orthopaedic Surgery and include Dr. Grimm, Dr. Bradley Peck, Dr. Bruce Klein, and Dr. Robert Meyer. Our experienced certified Physician Assistants are Marty Kenney, Jeff Trumbull, Aaron Bishop, Robert Feinman, Kevin Stewart, and Tim Jensen.
